Updated version fixed the low fps problem for me.
ExpandSystemMemoryX64=true
ReduceSystemMemoryUsage=true
DisableDriverMemoryManager=true
DisablePreloadToVRAM=true
ReservedMemorySizeMb=360
EnableUnsafeMemoryHacks=false
Test 1 Updated 0.201 binary, vsync on, all other settings as previous
Had a couple of second freeze part way through, where previously the game has sometimes frozen, but it recovered on this run
Test 2 same settings - no freeze this time
Update runs much smoother, much less stutters after initial load and no fps drops

Test again many times this test from my latest posts and version which on site now, i need to be 100% sure, because what i did is just tried to put manager updating before end of frame instead of after, so if results are corrent for you between many tests, then it's drivers.

3 Test in riverwood
3 Test outside of whiterun
Before i had super slowdown standing at the place where the first dragon attack and camera focused at whiterun+the mountains.
Now only small micro stutter one time thats it.
In riverwood i had super slowdown at the 3 magic stones, camera focused at the lake or towards riverwood.
Now when i look at the lake one micro stutter then no more lag, when i change focus and look at riverwood one micro stutter then no more lag
Conclusion - i can play the game without any need to purge VRAM

I'm not tweaking everything without knowledge, i just set textures to medium via settings, because thats what the parameter does 0 high, 1 medium, 2 low
so you are telling me i have to have texture set to high ? hm ... wonder why .200 is ok then XD

Longer more varied run, visited lots of dense locations, lots of combat in dense areas - forest just outside of Riften with Flora Overhaul Summer + Lush trees and grass lots of combat, lots of large textures: no freezes, no large fps drops and no crashes, even survived my 4k giants with no fps loss. Lots of fast travels without infinite loads. No noticeable FPS loss when VRAM maxed out.

ForceAnisotropicFiltering=true
MaxAnisotropy=16
is working for you??, while ingame (skyrimprefs) and drivers filtering is disabled (negative lodbias set to clamp)
