I think it may just be an issue with the DoF filter I use. It came from one of the
Grim & Somber ENB presets and is pretty old.
As per mindflux' suggestion, I replaced the fx file with the
optional enbdepthoffield.fx that comes with
Rudy ENB for CoT and this filter does
not exhibit the issue. Flames are nicely blurred when out of focus

Interesting. The
default DoF filter from
Rudy ENB 319 for CoT does show the problem a little bit, but only when the flames are far away and viewed at a certain angle.
