ctd @ riverwood entrance so some steps further into then the ctd occured before
Helgen Keep had some of it's bottleneck scenes back that where much better handled latency wise with ReduceSystemMemoryUsage=true
Page faults where extremely higher almost 3x as high as with ReduceSystemMemoryUsage=true
that surely explains the not so good latency and the bottlenecks

For my AMD 7850 2GB, only x64 flag to true was stable. Even with this setting, v0.195 appears to be much smoother as I ran. A good progress my point of view.

When I had T, F, T, T, panning camera side to side was quite stuttering at half stump tree next to Riften Snow-shod farm.
When I had T, F, T, F, a micro stuttering at same place but got the R6025 pure virture call error when trying to exit the game. I had to TaskMgr to get rid of black screen by killing Skyrim.
T, F, F, F had much less stutter than T, F, T, F.

Maybe I am annoying, but I am wondering if it'll be not a good idea to add (in a future version) an optimized Motion Blur as extra effect.
With this quite beautiful effect, it would be possible to use adaptive quality without perceptive visual loss during fast camera moves.

this is definitely the most stable on my Nvidia Intel system
though it shows the severe bottleneck issues in interiors (mostly on portals but also other places), but it didn't ctd for a much much longer period
so the question is now what option or which combination now keeps the ctd away and the bottlenecks on the lowest level (page faults)
also took some very nice shoots of the details i was able to render with this on my rather limited system (at least compared to many others here )
will post them in the Gallery

Updated to this and I am using Real Vision Beta 195 from Nexus..
Still getting Black Screen/Freeze and an occasional CTD but not as often as other ENB's.
Heavily modded with HD textures, used the texture compression tool on them though.. can't tell if that actually helps or not..
i7 3770K @4.6GHZ 2xAsus 660Ti TOP cards in SLI @5040x1050 Resolution.. (nvidia surround)
Frame rate is all over the place 15-60 FPS..

@Boris - With ReduceSystemMemoryUsage set to true I get major in game frame stutters when running around in exterior areas. I only just now am reporting this due to having had taken a break from Skyrim over the past few weeks. Not sure if this is a known issue, unknown issue or just an issue specific to my custom preset. Setting it to false completely fixes the frame stutters.

If not, I have a personal request : (won't be surprising coming from me )
I don't know the hell how your algorythms are actually working, but :
Can you make " -1 " quality level available for each quality var if they aren't yet ?
Can you set a " -2 " value for extreme quality for effects that would benefit from it ?
Do you have enough wide range in computations for the effects to yet increase their quality ?
On powerful rigs, performance differences between 0 and -1 is not that heavy, I would sign for a "no prisoner" extreme quality setting right away.
